Please read the general questionnaire. It can apply to you as an individual or to your child.
     
  1. Do you feel that you have to work harder than anyone else in school and yet can't get the marks?

Do you feel your child is underachieving in school?
     
  2. Do you feel stupid, or does your child think he/she is stupid?
     
  3. Do you feel that you can't get your thoughts organized?

Does your child have problems with organization?
     
  4. Do you have difficulty reading and/or understanding written text?
- or -
Have you noticed that your child has difficulty reading books
or is reading books below his/her grade level?
     
  5. Do you or your child have difficulty listening, remembering or concentrating?

Has there been a change in attitude toward school or has your child recently exhibited negative behaviours either in school or at home?
If you have answered "yes" to any of the above six questions, then continue on with the reading and math questionnaire.
     
  Reading and Math Questionnaire
  Do you or your child:
  1. make wild guesses when reading unknown words?
     
  2. substitute similar words that start with the same letter? (reads "stop" instead of "step" or "money" instead of "monkey")
     
  3. reverse the letter-sequences of whole words (reads "was" instead of "saw") or shuffle the order of letters inside a word (reads "blub" instead of "bulb")?
     
  4. reverse letters and/or numbers (saying "76" for "67" or "b" and "d" confusion)?
     
  5. shorten long words into shorter ones (reading "elope" instead of "envelope" or "computer" instead of "contemporary")?
     
  6. substitute words that have similar meanings (reading "house" instead of "home" or "long" instead of "length")?
     
  7. have trouble remembering new words (learning a new word on the first line of a paragraph but forgetting it before the last line of the same paragraph)?
     
  8. hold a book upside down without noticing?
     
  9. find remembering math facts (addition, subtraction, multiplication, division) a problem no matter how much you or your child practice?
     
  10. do poorly on math tests even though you know the work?
